Step 2: If you do not have access to a printer, click the below REQUEST FORM box, complete the required information on the Request Form, and then click the submit button. Upon receiving your request, VasoCARE will mail you a blank Prescription/Rx form. Once completed and signed, please fax the completed Prescription/Rx form back to VasoCARE 1-866-455-5150.
